For VA facilities, families & referring teams

Getting a Veteran to the right care should not depend on geography.

CCC helps VA teams, Veterans, and families coordinate the practical handoffs between assessment, travel, residential care, discharge, and return home.

The complete pathway

Emergent care does not begin at our front door — and it does not end at discharge.

01Access

CCC helps identify the appropriate referral, authorization, or emergency pathway and who needs to be involved.

02Assessment

Clinical assessment confirms risk, treatment needs, medications, co-occurring conditions, and whether residential care is appropriate.

03Travel to Care

CCC coordinates with the referring team on safe transport, arrival timing, records, medications, and any VA-approved travel arrangements.

04Residential Treatment

During treatment, CCC maintains necessary clinical communication and begins discharge planning early.

05Return Travel

Before discharge, return travel, medications, follow-up appointments, housing, safety planning, and the next clinical contact should be clear.

06Benefits & Reintegration

CCC helps connect the Veteran back to VA care, family, housing, benefits, peer support, and ongoing behavioral-health treatment.

Emergent suicide care

If the Veteran is in immediate danger, safety comes before logistics.

Emergency departments and community providers treating an eligible Veteran should follow current VA emergency-care and COMPACT Act reporting requirements. VA directs providers to report emergency care through the Emergency Care Reporting portal or by calling 844-724-7842 so VA can begin eligibility review, care coordination, or transfer when appropriate.

One Veteran. Multiple systems. One plan.

Where the Veteran lives should not determine whether the handoffs work.

If a VA facility, clinician, Veteran Service Organization, hospital, family member, or other authorized partner is trying to place a Veteran into appropriate residential care, contact CCC early. We will help identify what must happen next and who needs to be involved.
